Due to the Covid-19 pandemic we have decided to temporarily suspend the online appointment system. The facility to order prescriptions online has not been affected. The sit and wait blood clinic has also been suspended and will operate on a pre booked appointment basis. We apologise for any inconvenience this may cause and appreciate your patience during this difficult time.

New Patient Registrationregister_now

If you are within the catchment area for Castle Healthcare Practice and would like to register please download the relevant forms below (you need two forms for each person registering - 1 - the Adult or Child form and 2 -the family doctor registration form) fill in and bring into the Practice.

Please bring your NHS number, some photo ID and proof of your address (Utility bill, bank statement etc) to reception. .

You will also need to fill in an NHS family doctor services registration form - please click here This form may also be used if you wish to register as an organ and/or blood donor.

NB:
You must supply: Photo ID & Proof of address

If you are taking any regular medication you will need an appointment to speak the doctor before a repeat prescription can be issued.

We are required to provide a named and accountable GP for all patients.
Yours is the Registered and Usual GP automatically allocated when registering at Castle Healthcare Practice. If you require more information please ask at reception.  This may not necessarily be the GP that you choose to see.

If you move outside of our practice catchment area you will be sent a letter asking you to re-register at a practice closer to your new home.

Changes to Personal Details

Please notify the receptionist of any change of address or telephone number. Please also let us have your mobile telephone number.  If you give consent for SMS messaging, we can contact you to send appointment reminders and other messages.

We will never send confidential patient data by SMS message.
